"_____ It's time for your daily therapy." Says the nurse, unlocking the door to my cell. I slowly stand up, and as he walks over to me, I dash out past him. "We've got a runner!" He shouts, the sirens coming on. I loosen up my straightjacket and slip it off. As I'm about to reach the doors, I'm caught by a doctor, who picks me up swiftly by the arms.

This little (yeah right) tidbit of a horror story is for 's Halloween contest. Their theme is 'Scary Halloween.' It's been awhile since I've attempted horror (probably ten+ years), so I don't know if this is actually "scary" or just demented. I tried to add horror aspects, but I don't know how well I succeeded...
There's quite a bit of background behind this jewel, and I'd like to share a paraphrased summary of it with you~
The story takes place in an abandoned mental asylum -- Blackbriar -- which is a play off the name of a certain album by a certain band (Nox Arcana's "Blackthorn Asylum," for those who are interested). The torture the characters experience are actual "treatments" asylums used over the years, but characterizes and intensifies the suffering to fictional(?) levels.
Infection Cutting
The first treatment done on Fem!Austria/Richelle was first conceived by Dr. Henry Cotton at the New Jersey Trenton Psychiatric Hospital in the early 1900s. He believed infections caused insanity, and so by removing the infection, insanity inducing bacteria is also removed and the insanity should disappear. He would pull patient's teeth, remove their tonsils, and remove pieces of their reproductive and digestive organs.
Electroshock Therapy
The treatment Germany/Ludwig is forced to endure is one of the more well-known treatments though nowadays it's making a comeback in modern society under the name "electroconvulsive therapy." Otherwise known as ECT, the shock treatment was mainly used for patients experiencing schizoprenia. Resulting consequences of ECT involves memory loss and confusion.
Lobotomy
The second well-known treatment Hungary/Elizabeta experienced is a lobotomy. Lobotomies involve severing areas of the brain to produce a calming effect in the patient. A Portuguese neurologist, António Egas Moniz, first dabbled in the idea of lobotomy and created the specific lobotomy I used in my story -- the transorbital lobotomy. He took an icepick from his pantry, a mallet, and proceeded to perform a frontal lobe lobotomy through a patient's eye socket by breaking the orbital (eye) bone and scraping away parts of the patient's brain.
Bloodletting
Fem!Italy/Felicia's treatment is bloodletting. This one has been around since ancient times, though it's been modified for the time it's used in. Doctors could use anything ranging from knives to leeches to the device the nurse turned against both Felicia and Henry -- a scarificator (which is a nasty looking device, I suggest looking it up). Bloodletting was used for a number of diseases.
Immersion Therapy
What Prussia/Gilbert deals with is something called immersion therapy, which is something personally new to me after so many times researching asylums. Doctors would place patients in either warm or cold water depending on their temperaments and illnesses. For example, someone who's lethargic would be placed or doused in cold water while excitable patients were placed or doused in warm water. Some patients were even wrapped from head to toe in towels soaked in water and left in them for several hours.
Solitary Confinement
The coup-de-grace, the climax, of the story pertains to Male!Hungary/Henry's descent into madness and solitary confinement. Solitary confinement is used as a protection treatment -- the patient is dangerous either to him or herself and/or others, and so needs to be separately confined. In the cases they're dangerous to themselves, they may be put into a straightjacket to keep themselves from harm.

High upon the rocky cliffs overlooking the Miskatonic River, the hollow edifice of Blackthorn Asylum stands as a bleak monument to the horrors that once lurked within its desolate halls. Originally built as a sanitarium for the criminally insane, the asylum gained a grim reputation after dark rumors began to spread throughout the area. Whispered tales told of horrible experiments conducted upon Blackthorn's most violent and deranged psychopaths in an attempt to extract the essence of evil.
Over the decades, the torments inflicted upon the inmates resulted in unlocking the demons from their minds, but as a horrific consequence, the tortuous experimentation also unleashed the vile malediction that now infests the abandoned corridors.
